Residential Access Form

Access to your premises:

You authorize South Central Indiana (SCI) REMC d/b/a SCI FIBER and its agents to install a fiber drop at the address indicated in the registration process and to turn on residential fiber services.
You warrant that you are either the owner of the premises or that you have the authority to give SCI REMC perpetual access to the premises. If you are not the owner of the premises or do not have authority, you are responsible for obtaining necessary perpetual approval from whomever does have authority to allow SCI FIBER and its agents onto and into the premises to perform the activities to install, configure, maintain, inspect, upgrade, replace, or remove the services and/or equipment used to receive any services. If requested, you agree to supply SCI FIBER and its agents the owner’s name, address, and phone number and/or evidence that the owner has provided such authorization.


SCI REMC will notify you before it begins the installation of your fiber drop. Once your fiber drop installation is complete, SCI REMC or its agents will contact you to schedule an in-home appointment to complete service installation, after which you agree to start receiving broadband services from SCI REMC for a minimum of one year. If you cancel your service after the fiber drop installation begins and before the end of the one-year term or do not take service within 30 days of the drop being completed, SCI REMC reserves the right to charge you the drop installation costs.
